Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana State Physical Therapy license is required to start the assignment.
• BLS Certification from AHA or ARC may be required.
• Outpatient physical therapy experience preferred, including evaluation, plan of care development, and functional treatment planning.
• Experience with documentation of evaluations, goal writing, letters of medical necessity, and charge entry in compliance with payer and clinic policies preferred.
• Ability to supervise and delegate to support staff and to participate in student clinical education preferred.
• Maintain professional licensure and complete state-mandated continuing education. Adherence to state Practice Act and professional code of conduct required.
