FL lic required
Number of beds in your facility? 40
EMR for your facility: Protouch/ Net Health

Required Qualifications
Physical Therapist, Long Term Acute Care
Experience: 1 year
Licenses: PT-FL
Certifications: BLS-AHA(Copy needed)
SSN Required
DOB Required
Work History: Should not be more than 90 days of gap in last 1 year
