Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana Physical Therapist license required prior to the start of the assignment.
• BLS certification from AHA or ARC may be required.
• Experience providing home health physical therapy required. Client prefers clinicians with documentation experience using Home Care Home Base.
• Experience with productivity expectations and point-based productivity systems preferred (client productivity requirement: 30 points per week, 60 points per two-week pay period).
• Ability to travel within assigned counties for patient visits including Marion, Hamilton, Hancock, Shelby, Johnson, Morgan, Hendricks, Boone, and Madison counties.
• Ability to perform comprehensive PT evaluations, develop measurable goals and treatment plans, supervise PT assistants and aides as allowed by state practice act, and maintain required documentation in company and regulatory formats.

• Shift name: Days. Shift schedule: Monday through Friday, day shift.
• Work pattern: 5x8 schedule.
• Mileage reimbursement: $0.41 per mile for patient-to-patient travel. No reimbursement for home to first patient or last patient to home.
• Counties covered: Marion, Hamilton, Hancock, Shelby, Johnson, Morgan, Hendricks, Boone, Madison.
