Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Michigan Physical Therapist license required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Must have at least 12 months of Home Health physical therapy experience within the past 3 years. (Client-specific requirement)
• Experience with HCHB electronic medical records required. (Client-specific requirement)

Assignment Details
