Minimum Requirements:
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.
- Active Washington State Physical Therapist license is required to start the position.
- BLS Certification may be required from AHA or ARC.
- Clinical experience in home health or comparable outpatient/rehabilitation settings is preferred.
- Ability to complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition.
- Experience appropriately utilizing ICD-10 codes.
- Experience directing or supervising Physical Therapy Assistants and Home Health Aides, as assigned.
- Willingness to travel and provide care across the greater Seattle area (coverage excludes west and south Seattle). Primary coverage includes North Seattle and Shoreline.

Responsibilities:
- Provide physician-ordered plan of care under the direction and supervision of the Director of Clinical Services.
- Work in conjunction with all home care personnel in patient care planning.
- Direct and supervise Physical Therapy Assistants and Home Health Aides, as assigned.
- Take initial patient history and perform initial evaluations.
- Consult with physicians regarding changes in treatment.
- Instruct patients and family in home programs and activities of daily living.
- Participate in Quality Assessment and Performance Improvement activities as assigned.
- Complete required documentation and coding, including appropriate use of ICD-10.
- Manage a typical caseload of approximately 25 visits when working independently; typical caseload is approximately 20 visits when working with a PTA.
