Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana Physical Therapy license is required to start the assignment.
• BLS certification from AHA or ARC required.
• Minimum 1 year of outpatient physical therapy experience is required. This position is primarily outpatient with approximately 5-10% inpatient care.
• Experience with outpatient orthopedic caseloads preferred. Approximately 70% of outpatient diagnoses are orthopedic.
• Experience with equipment such as Hivamat, therapeutic ultrasound, electrical stimulation, BITS, and laser is preferred.
• Familiarity with Epic EMR preferred.
• Manual therapy skills preferred.

Assignment Details

• Facility Type: Hospital - Physical Therapy department (primarily outpatient).
• Shift Name: Days.
• Shift Details: Days 08:30-17:00, Monday through Friday.
• Weekly Guaranteed Hours: 36.
• Patient Ratio: Approximately 1:1 in unit setting.
• Required Certifications: BLS and current state licensure.
• Float Requirements: None.
• Dress Code: Charcoal grey scrubs. Denim is not acceptable attire.
• First Day Instructions: Park on the south side of the outpatient building at 1810 Lafayette Rd. Enter through the front doors. Complete any assigned HR education modules prior to the first day.
