Job Overview
We are seeking a skilled Physical Therapist (PT) to provide high-quality rehabilitative care within a skilled nursing and rehabilitation setting. This role is responsible for evaluating patients, developing individualized treatment plans, and delivering therapy interventions to improve mobility, strength, and functional independence.
The position requires strong clinical judgment, collaboration with interdisciplinary teams, and the ability to support patients across a diverse rehab population.
Key Responsibilities
Evaluate patients physical conditions and functional abilities
Develop and implement individualized treatment plans
Deliver therapeutic interventions to improve mobility and strength
Document patient progress and adjust treatment plans as needed
Collaborate with physicians, nurses, and rehab staff
Educate patients and families on recovery goals and exercises
Ensure compliance with clinical standards and regulatory guidelines
Maintain accurate and timely clinical documentation
Support discharge planning and continuity of care
Promote patient safety and functional independence
Job Requirements
Active Physical Therapist (PT) license (state of California or compact/eligible where applicable)
Doctor of Physical Therapy (DPT) or equivalent degree
Previous experience in skilled nursing, rehab, or acute care preferred
