Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana Physical Therapist license required to start the assignment.
• BLS certification from American Heart Association or American Red Cross required.
• Minimum education requirement: Bachelor’s degree required, PT state licensing required.
• Experience: open to all levels; preferred outpatient orthopedic experience.
• EMR experience: familiarity with Epic preferred.
• Client-specific requirements: ability to provide one-on-one outpatient orthopedic treatment, wear gray scrubs or facility-branded apparel, complete contract personnel requirements prior to start.

• Facility Type: Hospital outpatient clinic (Outpatient Orthopedic Rehabilitation).
• Shift Name: Days.
• Shift Details: Monday–Friday, 08:00–16:30.
• Weekly Guaranteed Hours: 36.
• Call Requirements: none.
• Patient Type: general outpatient ortho, one-on-one treatments, may include walk-in patients.
• EMR: Epic.
• Dress Code: gray scrubs or facility-branded apparel required.
