Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapist license required to start the assignment.
• BLS certification required from AHA or ARC.
• Experience providing outpatient physical therapy with primarily 1-on-1 care.
• Experience documenting in Epic or similar electronic medical record systems.
• Experience treating adult and adolescent caseloads including orthopedic and neurologic diagnoses. Client notes indicate outpatient caseloads in a rural health setting with ages approximately 10–100.
• Ability to supervise and collaborate with physical therapy assistants.
• Strong clinical skills, customer service, documentation quality, time management, and teamwork.
• Preferred: Experience floating to inpatient settings occasionally.

Assignment Details

• Facility Type: Hospital (Outpatient clinic with occasional inpatient float).
• Shift Name: Evenings.
• Shift Details: 36-40 hours per week; Monday–Friday with evening hours. Schedule options include 3x12, 4x10, or variable 8s to be determined with the candidate.
• Clinic Hours: 7:00 AM–7:00 PM.
