Travel Physical Therapist (PT) - $2,105 to $2,305 per week in Surprise, AZ

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• 1+ year of PT experience required
• No new graduates

Assignment Details

• Assignment length: 13 weeks
• Start 2+ days per week at 7am and end 2+ days per week at 7pm, with the last patient scheduled at 6pm
• Clinic hours are Monday 10am-7pm, Tuesday 7am-4pm, Wednesday 10am-7pm, Thursday 7am-4pm, and Friday 7am-4pm
• Expected caseload is 55-65 visits per week
• Patient volume is typically 11-13 patients per day on an 8-hour schedule
• Evaluation visits are 60 minutes and follow-up visits are 30 minutes
• Documentation system includes NextGen and Sidekick AI documentation
• Possible floating to another clinic approximately 15 minutes away
• Dress code is business casual
