Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky Physical Therapy license required to start the assignment.
• BLS certification from AHA or American Red Cross may be required.
• Must have at least 12 months of Home Health physical therapy experience within the past 3 years. (Client requirement.)
• Experience with HCHB (Homecare Homebase) EMR preferred.
• Ability to work in a hospital-based home health territory and follow agency policies.

Assignment Details

• Facility Type: Hospital (Home Health service line).
• Shift Name: Days.
• Shift Details: Monday–Friday, 8:00 AM–5:00 PM, 8-hour days.
• On call: One weekend per month on call.
• Length of Contract: 13 weeks.
• Positions Available: 1.
