This role requires a proactive professional to perform physical therapy evaluations and treatments. Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluations and treatments based on physician orders/referrals
  • Use appropriate evaluation and assessment tools and procedures
  • Review patient records for diagnosis, medical history, and consider precautions and contraindications
  • Develop and implement treatment plans with measurable goals
  • Re-assess treatment plan effectiveness regularly and modify as needed
  • Communicate patient needs and progress to the treatment team and relevant parties
  • Consult with medical providers regarding evaluations, treatment, and discharge planning
  • Recommend and assist in obtaining adaptive equipment for patients
  • Maintain and submit documentation as required
  • Adhere to professional practice standards and regulations
  • Supervise Physical Therapist Assistants and Aides
  • Participate in or conduct internal staff development programs and obtain continuing education

Requirements of the Physical Therapist
  • DPT License
  • Proficiency with HCHB electronic charting system
  • Knowledge of disease processes and patient education
