This role focuses on home health physical therapy services, utilizing specialized skills to provide patient care based on physician orders. Key Responsibilities of the Physical Therapist- Perform physical therapy evaluation and treatment based on physician orders/referrals
- Complete comprehensive evaluations with the appropriate tools and procedures
- Review patient records for diagnosis, history, precautions, and contraindications
- Establish goals and treatment plans for patient independence/function
- Re-assess treatment plan effectiveness and patient progress
- Communicate patient needs and progress to treatment team and family
- Consult with medical providers regarding patient evaluation and discharge
- Recommend and secure adaptive equipment for patients
- Maintain and submit required documentation following regulations
- Supervise Physical Therapist Assistants and Aides in patient care
- Attend and participate in staff development and educational programs
Requirements of the Physical Therapist- Valid Physical Therapy license in the state of Arkansas
- Experience with HCHB and OASIS
- Home Health experience
