Job Title: Physical Therapist Profession: Physical Therapist (PT) Specialty: Long Term Care (LTC) Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum of 1 year of PT experience preferred; New graduates with Level II experience will be considered License: Missouri State License required Must-Have: LTC or Rehab experience Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluation and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all required information in the electronic documentation system and maintains patient records according to policy. Accurately records treatment minutes for services provided per regulatory and clinical requirements. Participates in required meetings, including patient care conferences and rehabilitation meetings. Engages in facility and company-required training and in-services. Able to obtain necessary durable medical equipment or supplies per regulatory and clinical practice requirements. Supervises and supports physical therapist assistants, rehabilitation aides, and/or students. Adheres to practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
