Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Colorado Physical Therapist license required prior to the start of the assignment.
  • BLS certification from AHA or ARC may be required.
  • Minimum of recent home health experience preferred. (Preferred)
  • Experience with Homecare/Home Health electronic documentation systems required; HCHB experience preferred. (Preferred)
  • Ability to meet a productivity requirement of a minimum of 30 visits per week as specified by the client.
  • Valid driver’s license and reliable vehicle to travel within assigned counties (Mesa County and Garfield County).


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Monday through Friday, 8:00 AM – 5:00 PM.
  • Counties Covered: Mesa County and Garfield County; travel between patient homes within these counties is required.
  • Supervisory Responsibilities: May supervise Physical Therapist Assistants and Physical Therapy Aides in accordance with state practice act.
  • Documentation: Timely completion and submission of evaluation, treatment, and billing documentation per company and regulatory requirements.
