Minimum Requirements

  • Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Ohio PT license required to start the assignment.
  • Minimum one year of home health (HH) physical therapy experience required.
  • Experience documenting in HCHB (Homecare Homebase) EMR required.
  • BLS certification from AHA or ARC may be required.
  • Ability to perform Start of Care, recertifications, evaluations, and treatment visits in the home health setting.
  • Preferred: Lymphedema therapy experience.
  • Dress code: scrubs.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Days 0800-1600.
  • Weekly Hours: 40 (weekly hour guarantee).
  • Coverage Areas: Travel between counties including Pickaway, Fairfield, Ross, Muskingum, and Licking as assigned.
  • Mileage reimbursement provided between visits from first patient to last patient.
