Description
We are seeking a compassionate and dedicated Travel Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF). The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide evidence-based therapy services to improve mobility, strength, balance, endurance, and overall functional independence.

The ideal candidate is adaptable, patient-focused, and committed to delivering exceptional rehabilitative care to residents recovering from illness, injury, surgery, or age-related conditions.
Responsibilities
Perform comprehensive physical therapy evaluations and develop individualized treatment plans
Provide therapeutic exercise, gait training, balance training, transfer training, manual therapy, and functional mobility interventions
Assess patient progress and modify treatment plans to maximize rehabilitation outcomes
Educate patients, families, and caregivers on home exercise programs, mobility techniques, fall prevention, and safe transfer strategies
Complete accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory requirements
Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary healthcare team
Participate in interdisciplinary care planning and discharge planning
Maintain a safe, clean, and organized treatment environment
Ensure compliance with all federal, state, and facility regulations
Deliver compassionate, patient-centered care focused on restoring function, promoting independence, and improving quality of life
Requirements
Required: Graduate of an accredited Physical Therapy program
Required: Active Physical Therapist (PT) license in the state of assignment or ability to obtain
Preferred: Minimum of 1 year of Skilled Nursing Facility (SNF) or post-acute care experience
Strong communication, documentation, organizational, and interpersonal skills
Ability to adapt quickly to new clinical environments and work independently
