Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Indiana Physical Therapist license is required to start the assignment.
  • Current PT professional certification as required by state (PT).
  • Home health physical therapy experience required. Experience with start of care, resumption of care, discharges, and oversight of PTAs preferred.
  • Familiarity with home health EMR systems. Experience with HCHB strongly preferred.
  • Experience using electronic medication administration records (eMAR) systems preferred.
  • BLS certification may be required from AHA or ARC.
  • Minimum education requirement: Bachelor's degree.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Monday–Friday, 0800–1630 (daily hours include patient visits within this timeframe).
  • Weekly Hours Guarantee: 36 hours.
  • Productivity expectation: 30 points.
  • Coverage Areas: Marion and Hamilton counties.
  • Mileage reimbursement between patient visits from first to last patient: Yes.
  • On-call/weekend requirement: None.
  • Primary responsibilities: PT home health visits, start of care, resumption of care, discharges, and oversight of PTAs.
