Minimum Requirements

  • Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by APTA.
  • Active Florida Physical Therapy license required.
  • HCHB and OASIS proficiency required.
  • Experience in home health or hospice settings preferred.
  • BLS certification from AHA or ARC may be required.
  • Experience conducting in‑home evaluations, home safety assessments, and developing individualized Plans of Care preferred.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Evenings.
  • Shift Details: Evenings 2000-0400.
  • Days: 5x8 schedule.
  • Weekly Hours Guarantee: 40 hours.
  • Coverage Area: Orange County, primarily east and central Orlando; travel between patient visits with mileage reimbursement between first and last patient visits.
  • On Call/Weekend Requirement: No.
  • Productivity Expectation: 25 points.
  • Must manage assigned caseload independently and meet schedule and timeliness expectations.
  • Interdisciplinary collaboration with nursing, OT, ST, social services, and physicians required.
  • Florida PT license is required prior to start.
