Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Current Physical Therapy licensure in Washington state or a compact license with privilege to practice in Washington.
  • Current CPR certification.
  • Valid driver’s license, vehicle insurance, and reliable vehicle for patient visits.
  • Experience providing home health physical therapy, including patient evaluations, development of plan of care, and CMS documentation.
  • Ability to complete progress notes per agency policy and prepare discharge summaries at required intervals.
  • Willingness to travel throughout Pierce County for patient visits and to cover other nearby agency service areas as needed.
  • Must meet client-specific expectations: ability to meet productivity targets (30 points) and a weekly hour guarantee of 36 hours.
  • Minimum education requirement: Bachelor’s Degree.
  • Preferred: experience with HomeCare HomeBase electronic documentation.
