Seven Healthcare are seeking a dedicated Physical Therapist specializing in Home Health for a travel assignment in Carmel, Indiana. This role involves providing comprehensive physical therapy evaluations and treatments for patients in various counties, including Marion, Hamilton, and Hancock, among others.

Key Responsibilities of the Physical Therapist - Travel
  • Perform physical therapy evaluation and treatment based on physician orders/referrals.
  • Conduct comprehensive evaluations using appropriate tools and procedures.
  • Review patient records for diagnosis, medical history, precautions, and contraindications.
  • Establish measurable goals and develop treatment plans to enhance patient independence and function.
  • Regularly re-assess treatment effectiveness and progress towards goals.
  • Communicate patient needs and progress to the treatment team and medical providers.
  • Consult on patient evaluation, treatment progress, and discharge plans.
  • Recommend and assist in acquiring adaptive equipment to enhance rehab programs.
  • Maintain and submit required documentation as per regulations and company standards.
  • Supervise Physical Therapist Assistants and Aides in patient care activities.
  • Participate in staff development and continuing education programs.

Requirements of the Physical Therapist - Travel
  • PT license in Indiana
  • Experience in Home Health and with Home Care Home Base
  • Proficiency in OASIS documentation
