Job Title: Physical Therapist - Home Health Profession: Therapy/Rehabilitation Specialty: Physical Therapy Home Health Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 17:00 Experience: Minimum of 1 full year of Adult Home Health experience License: Must have license in hand, no exceptions Certifications: CPR, BLS Must-Have: Recent Home Health experience, reliable transportation, current driver's license, and agency-required liability insurance Description: Home visits for therapy services will be performed. Periodic office meetings will be conducted. Must have reliable transportation. The average number of patients seen per day is 5-6. Contractors will be assigned between 5-10 patients per day. Each visit and activity such as documentation and travel hold different point values. The contractor is expected to meet a productivity requirement of 30 points per week. Types of cases include Start of Care and Follow Up visits. Weekend requirements vary by location and profession. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Contractors are compensated for travel between patients' homes. Commute to and from the first and last patient is not billable. Local applicants are accepted at the same rate. The contractor must understand that they will be responsible for driving to patients' homes daily. The contractor must confirm their vehicle is operational and that they have insurance. Longer drives between patients may be required. The contractor must accept every patient on their schedule regardless of distance. All documentation must be completed within 24 hours of patient appointments. There are no guaranteed hours with this contract. The contractor must achieve 30 productivity points weekly. All overtime requires pre-approval. If insufficient patients are received to meet financial needs, contractors will have the option to float to another nearby clinic.
