Travel Physical Therapist (PT) - $2,286 per week in Franklinton, NC

Job Title: Physical Therapist (PT) Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health experience required License: Must have a valid license Certifications: CPR, BLS Must-Have: Recent Home Health experience is required Description: Provide physical therapy services to patients in their homes. Conduct home visits and participate in periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients seen per day is 5-6. Points system will assign varying values to visits and other activities such as documentation and travel. The contractor will be assigned between 5-10 patients per day to meet productivity expectations of 30 points per week. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Commute to and from the first and last patient is not billable. Weekend requirements may vary by location and profession. On-call requirements will be discussed during the interview. Candidate will provide services to assigned counties. Documentation must be completed within 24 hours of patient appointments. There are no guaranteed hours; productivity expectations must be met. Declining patients may result in a cancelation. All overtime must be pre-approved. The candidate may have the opportunity to float to another nearby clinic if necessary. Travelers must be proficient in HomeCare HomeBase EMR and have used it within the last 6 months. No contract hours are guaranteed.
