Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program.
• Active Indiana Physical Therapy license required to start the assignment.
• Minimum of two years recent home health physical therapy experience is required.
• Proficiency with Homecare documentation systems, specifically HCHB and OASIS, is required.
• BLS certification from AHA or ARC may be required.
• Ability to perform comprehensive PT evaluations and develop measurable treatment plans, including supervising PTAs and aides, per state practice act.
