Minimum Requirements

  • Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctoral degree) approved by the APTA.
  • Current, active, and unencumbered Physical Therapist license in the state of Ohio prior to the start of assignment. State-specific licensure must meet requirements for assignment state.
  • Current Basic Life Support (BLS) certification from AHA or ARC.
  • Home health experience preferred. Candidates with experience in home visits, home safety evaluations, and developing home-based care plans preferred.
  • Ability to perform patient evaluations, develop and update plans of care, provide skilled physical therapy treatments, and document patient progress per physician orders and state regulations.
  • Supervisory experience preferred. Position may involve supervising and evaluating Physical Therapy Assistants and Home Health Aides as assigned.
  • Minimum 2-3 years of physical therapy experience preferred.
  • Reliable transportation and valid driver’s license required for home visits.
  • Proficiency with basic computer skills for documentation, e-mail, and online training platforms.


Assignment Details

  • Facility Type: Home Health.
  • Shift: Evenings (5x8 schedule).
  • Start/End Dates: Travel assignment with defined contract dates as provided by recruiter.
  • Responsibilities: Perform patient evaluations, deliver therapeutic services per physician orders, document care, evaluate home environment, and participate in interdisciplinary communication.
  • Supervisory Duties: May directly supervise and evaluate PTAs and Home Health Aides as assigned.
