Travel Physical Therapist (PT) - $2,326 per week in Lexington, KY

Job Title: Physical Therapist - Home Health Profession: Physical Therapist Specialty: Home Health Duration: 13 Weeks Shift: 8-Hour Days, Monday to Friday, with Rotating Weekends Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of 1 Year License: Required Certifications: BLS Must-Have: Home Care Home Base Experience and Home Health Experience Description: No clinical involvement. Profiles are sent directly to the manager for phone interviews. Travelers are required to perform "Start of Care," Recertifications, Daily Visits, and Evaluations. Common conditions treated include Congestive Heart Failure, Orthopedic Issues, and Pneumonia. Work areas cover Home Health. Experience with or skills in home health care preferred; no new graduates. Resources are available for staff to reach out to while in the field. Clinical Manager or Educator is available 24/7 for emergencies or from 8:00 AM to 5:00 PM. Work is typically scheduled for five 8-hour days. Weekend shifts are part of the rotating schedule and occur from 8:00 AM to 5:00 PM. Staff may be asked to participate in Holiday Rotation. Attire can be scrubs or business casual. Orientation includes two weeks with a preceptor and a few visits with a work coach. A phone and tablet will be provided for work purposes. The facility reserves the right to cancel shifts as specified. The charting system used is Homecare Homebase (HCHB).
