Seeking a dedicated and compassionate Physical Therapist (PT) for a contract opportunity in a rehabilitation and skilled nursing setting. The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide evidence-based therapeutic interventions designed to improve mobility, strength, balance, endurance, and overall functional independence.

REQUIREMENTS

• Active Physical Therapist (PT) License in good standing
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification preferred
• Previous experience in rehabilitation, skilled nursing, outpatient, acute care, or home health settings preferred
• Strong patient assessment and treatment planning skills
• Excellent communication, documentation, and interpersonal abilities
• Ability to work independently and collaboratively within an interdisciplinary healthcare team
• Commitment to delivering patient-centered care and positive clinical outcomes

KEY RESPONSIBILITIES

• Evaluate patients and develop individualized treatment plans
• Implement therapeutic exercise, mobility, balance, strength, and endurance programs
• Assess functional limitations and establish measurable rehabilitation goals
• Monitor patient progress and revise treatment plans as necessary
• Educate patients and caregivers regarding treatment plans and home exercise programs
• Maintain accurate and timely clinical documentation
• Collaborate with physicians, nurses, therapy assistants, and other healthcare professionals
• Participate in interdisciplinary care planning and patient progress reviews
• Ensure compliance with professional standards, regulatory requirements, and facility policies
• Promote patient safety, independence, and overall quality of life
