Travel Physical Therapist (PT) - $2,338 per week in Hawthorne, NY

Have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points. Types of cases include Start of Care and Follow-Ups. Must be willing to travel up to 75 miles from home clinic. Mileage is reimbursed at the IRS standard rate. Traveler is compensated from patient home to patient home. The commute to and from the first and last patient are not billable. Local candidates are accepted at the same rate. Travelers must be proficient in HomeCare HomeBase EMR and have used it within the last 6 months. There are no guaranteed hours with this contract. Clinician must meet weekly productivity points of 30, which equates to 40 hours per week. Expectations include accepting every patient on the schedule and completing all documentation within 24 hours of the appointment. Incomplete documentation that impacts billing and follow-up patient care may result in cancellation. If the contractor is not receiving enough patients each week to support their financial needs, they may have the opportunity to float to another nearby clinic to pick up patients.
