This role involves providing case management and direct patient care within a supportive team environment, delivering comprehensive physical therapy in the home setting. Candidates must have experience with Oasis and be comfortable working in a dynamic environment supporting the Finger Lakes Team.

Key Responsibilities of the Physical Therapist

• Develops and revises plans of treatments and discharge plans in coordination with patients, families, physicians, and care teams
• Initiates appropriate rehabilitative programs in the home setting
• Provides supervision of PTAs to ensure patient safety and compliance with care plans
• Consults and educates patients, families, and other team members on disease process, self-care and preventive strategies
• Monitors patient progress and adjusts physical therapy programs as needed
• Ensures completion of clinical records for compliance and service delivery
• Communicates effectively with patients, families, and care staff to promote positive interactions
• Supports care management philosophy for quality and cost-effective patient outcomes

Requirements of the Physical Therapist

• B.S., M.S. or D.P.T. degree in Physical Therapy
• Possession of a current New York State License in Physical Therapy
• BLS certification (AHA only)
• Minimum of one year experience in Long Term Care/Home Care setting as a Physical Therapist
• Recent Home Care experience within the last year
• Good verbal, written communication, and interpersonal skills
• Proficiency in physical therapy procedures
• Experience with Oasis is mandatory
