Job Title: Physical Therapist (PT) Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 year of Adult Home Health Experience License: Must have a valid license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Reliable transportation Current driver's license Agency-required liability insurance Description: Conduct home visits for therapy services. Attend periodic office meetings. Be prepared to travel up to 75 miles from the home clinic. Compensation for mileage will follow IRS standard rates. Daily patient load will consist of 5-10 patients. Expected productivity is approximately 30 points per week. Each visit and activity will hold a different point value for productivity tracking. Types of cases will include Start of Care and Follow-Up visits. Complete all documentation within 24 hours of the appointment. Declining patients may impact contract continuation. Accept all assigned patients regardless of distance from previous appointments. Understand that there are no guaranteed hours with this contract. Be open to floating to nearby clinics if necessary. Experience with HomeCare HomeBase EMR and OASIS is preferred. Travelers are encouraged to be comfortable with potentially longer commutes. Must be able to accommodate varied weekend requirements depending on location. Candidates with prior Home Health experience are preferred.
