Key Responsibilities of the Physical Therapist- Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for current diagnosis, past medical history, precautions, and contraindications
- Establish measurable goals and develop a treatment plan for patient independence and function
- Regularly re-assess treatment effectiveness and modify goals or treatment as needed
- Communicate patient needs and progress to the treatment team and appropriate parties
- Consult with medical providers on patient evaluation, treatment, and progress
- Recommend adaptive equipment to improve function and incorporate it into the rehab program
- Maintain and submit required documentation in compliance with regulations
- Adhere to professional standards and legal regulations
- Supervise Physical Therapist Assistants and Aides
- Participate in internal staff development and maintain clinical skills through continuing education
Requirements of the Physical Therapist- KY PT License, Compact License Accepted
- Experience with Home Care and Home Health
- Medicare Oasis experience
