Physical Therapist - Home Health - (PT - HH)

Pay Rate: $2472.00 - $2547.00
Travel PT Home Health jobs in Queens, NY are available for an ASAP start with a 13-week contract. You will work Monday through Friday, 8-hour days, seeing 5 to 6 geriatric clients daily in their homes.
You must have an active New York PT license and experience with the geriatric population. Home health experience is preferred but not required. Responsibilities include conducting home mobility assessments, developing individualized treatment plans, providing functional rehabilitation, and documenting care using Homecare Home Base. You will work independently, focusing on improving mobility, strength, and independence for clients in a community-based setting
