Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active California Physical Therapy license required prior to start of assignment.
• Current BLS (Basic Life Support) certification from the American Heart Association or American Red Cross may be required.
• Home health experience required. Must have documented visits providing evaluations, plan of care, and skilled interventions in home health settings.
• Minimum of 1 year of clinical experience as a licensed Physical Therapist preferred.
• Experience with electronic medical records and documentation in home health environments preferred.
• Valid driver's license, reliable transportation, and ability to travel within the assigned geographic region required.

Assignment Details

• Facility Type: Health System (Home Health setting).
• Shift Name: Days.
• Schedule: 5 shifts per week, 8 hours per shift.
• Weekly Hours: Approximately 40 hours per week.
• Start Date: June 2026, Duration: 13-week assignment.
• Must be able to provide care in patients' homes and perform travel between patient residences within the service area.
