Minimum Requirements
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent entry-level degree in Physical Therapy.
- Current State License as a Physical Therapist in the state of assignment is required to start the assignment.
- Proof of current CPR certification is required.
- Current driver’s license, reliable transportation, and active vehicle insurance are required.
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting is required.
- Experience with durable medical equipment and ordering appropriate rehabilitative equipment is required.
- Ability to perform physical demands of home health care including lifting, bending, pushing, pulling, prolonged standing and walking.
- Ability to work a flexible schedule and travel locally as needed.
- Must read, write, and speak fluent English.
- Previous home care experience is preferred.

- Scope: Provide evaluation, develop and revise Plan of Care, document progress notes and phone orders, and collaborate with interdisciplinary team.
- Patient Population: All age groups with neuromuscular, neurological, and orthopedic diagnoses.
- Clinical Responsibilities: Provide treatment, instruct on transfers and equipment use (walkers, wheelchairs, canes, braces), order and adapt rehabilitative equipment, provide patient/family education, and apply infection control practices.
- Documentation: Complete and maintain POC, progress notes, and other clinical records per agency policy.
