Job Summary (List Format):
- Evaluate residents mobility, strength, balance, gait, and functional limitations. - Develop and implement individualized, person-centered physical therapy treatment plans. - Deliver skilled PT interventions such as therapeutic exercises, gait training, and use of physical modalities. - Monitor resident progress and modify treatment plans to meet therapy goals. - Participate in interdisciplinary care team meetings and contribute to discharge planning. - Educate residents, families, and caregivers on home exercise and safety strategies. - Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides per licensure requirements. - Accurately document treatment sessions, progress notes, and billing information promptly. - Ensure compliance with state, federal, and facility documentation and practice standards. - Support quality assurance and performance improvement initiatives in the rehab team. - Work in a skilled nursing facility/long-term care setting, requiring frequent standing, bending, and lifting in a fast-paced, team-based environment.
