Job Summary (List Format):
- Evaluate residents mobility, strength, balance, gait, and functional limitations.
- Develop and implement individualized, person-centered physical therapy treatment plans.
- Deliver direct therapy interventions (therapeutic exercises, gait training, balance retraining, modalities).
- Monitor resident progress and modify care plans to meet therapy goals.
- Participate in interdisciplinary care team meetings and contribute to discharge planning.
- Educate residents, families, and caregivers on home exercise programs and safety strategies.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides.
- Accurately document treatment sessions, progress notes, and billing in compliance with regulations.
- Ensure adherence to all state, federal, and facility practice standards.
- Support quality assurance and performance improvement initiatives within the rehab team.
- Work within a skilled nursing facility (SNF) or long-term care setting, often requiring physical activity such as standing, bending, and lifting.
- Collaborate as part of a fast-paced, interdisciplinary, team-based rehabilitation environment.W
