- Evaluate residents mobility, strength, gait, and functional limitations through initial and ongoing assessments. - Develop and implement individualized, person-centered physical therapy treatment plans. - Provide direct therapy interventions such as therapeutic exercises, gait and balance training, and use of modalities. - Monitor residents progress, updating care plans as needed to achieve therapy goals. - Collaborate with interdisciplinary care teams and participate in discharge planning. - Educate residents, families, and caregivers on home exercise programs and safety strategies. - Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides, as required. - Accurately document treatment sessions, progress notes, and billing information in compliance with regulations. - Ensure adherence to all relevant state, federal, and facility documentation and practice standards. - Support quality assurance and performance improvement initiatives within the rehab team. - Work in a skilled nursing facility/long-term care setting, often requiring standing, bending, lifting, and assisting with resident mobility in a fast-paced, team-oriented environment.
