Travel PT Home Health jobs in Santa Rosa, CA offer a 13-week contract with Monday to Friday shifts from 8am to 5pm and 40 hours per week. You will provide physical therapy to patients in their homes, focusing on improving mobility and independence. Responsibilities include evaluating patient needs, developing treatment plans, and documenting care. Home health experience and a California PT license are required.
