Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active California Physical Therapist license required to start the assignment.
• Current BLS certification required from an approved vendor per facility policy.
• HomeCare HomeBase experience required.
• EPIC electronic medical record experience required.
• Valid driver’s license, current automobile insurance, and reliable automobile required. Must be at least 21 years of age.
• Prior home health or acute care physical therapy experience preferred.
• CPR certification or Basic Life Support (BLS or equivalent) is required.

Assignment Details

• Facility Type: Hospital (home health services managed through hospital-affiliated program).
• Shift: Days.
• Shift Hours: Monday–Friday, 0800–1700.
• Weekly Hours: 40 hours per week.
• Assignment Duration: 16-week travel assignment.
• Clinical Role: Manage a home health patient caseload independently and collaborate with interdisciplinary team members to meet care goals.
• Travel Requirements: Use of personal vehicle required for patient visits; mileage reimbursement provided.
