Description:
Provides physical therapy services to patients referred.
Includes assessment, treatment plan development, and implementation with follow-up and discharge planning.
Actively assists with clinical aspects of the department, including program development, department operations, and performance improvement activities.
Assists with supervising and coordinating student programs.
Supervises Licensed Physical Therapy Assistants, Rehabilitation Technicians, and Volunteers.
Performs comprehensive diagnostic evaluations to determine the status of physical and cognitive abilities of patients, including both inpatients and outpatients.
Considers diagnosis and age-specific needs in evaluations.
Develops and implements individualized treatment plans with patient and family participation.
Establishes measurable goals and methods to achieve those goals.
Adheres consistently to policies and procedures regarding documentation.
Participates in patient care and family conferences, in-service programs, and departmental meetings.
Counsels patients and families regarding evaluation, care, and treatment.
Initiates and completes discharge planning in a timely manner, providing effective education and training.
Coordinates and supervises student programs and volunteer initiatives.
Assists with orientation of new associates, patient scheduling, and assures quality coverage and appropriate staff supervision.
Demonstrates adherence to core values of the organization.
Supports and participates in performance improvement activities.
Develops efficient and creative ways to maintain departmental productivity standards.
Maintains awareness of community resources for appropriate referrals.
Identifies and provides educational opportunities for self, department, and community.
Participates in research activities as appropriate.
Demonstrates competence in performing assigned patient care responsibilities.
Adapts patient assessment, treatment, and care methods to meet individual needs.
Fosters positive and professional relationships with medical staff and referral sources.
Performs other related duties as directed.
