- Evaluate residents mobility, balance, strength, gait, and functional limitations through initial and ongoing assessments.
- Develop and implement individualized, person-centered treatment plans to restore/improve physical function, reduce pain, and prevent disability.
- Provide direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and application of physical modalities.
- Monitor resident progress and modify treatment plans to achieve therapy goals.
- Collaborate with the interdisciplinary care team and participate in care planning and discharge processes.
- Educate residents, families, and caregivers on home exercise programs and safety strategies to enhance independence.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides according to licensure requirements.
- Accurately document treatment sessions, progress notes, and billing information in compliance with regulatory and facility standards.
- Ensure adherence to state, federal, and facility documentation and practice guidelines.
- Support quality assurance and performance improvement initiatives as part of the rehabilitation team.
- Work in a skilled nursing facility (SNF) or long-term care environment, requiring frequent standing, bending, lifting, and assisting with resident mobility.
