Physical Therapist - Home Health - (PT - HH)

This Home Health Physical Therapist role is based in Denver, Colorado, a city known for its dynamic blend of urban energy and outdoor adventure. In this position, you will provide one-on-one physical therapy services in patients’ homes across the greater Denver area. You will play a key role in helping patients regain independence, improve safety, and remain in the comfort of their own homes. Typical patients include adults and older adults recovering from surgery, managing chronic conditions, or coping with mobility and balance challenges. Your day generally includes traveling between patient homes, performing comprehensive evaluations, and delivering individualized treatment sessions focused on functional mobility, gait training, strength and endurance, balance and fall prevention, and home safety. You will educate patients and caregivers on safe transfers, use of assistive devices, energy conservation, and home exercise programs tailored to each person’s goals and environment. You will work closely with an interdisciplinary team, collaborating with nursing, occupational therapy, speech therapy, and other disciplines to coordinate care plans. Communication is primarily through EMR and team meetings, allowing you to remain connected and supported even while working independently in the field. Documentation will be completed using an electronic medical record system with mobile access, enabling timely and accurate charting at or near the point of care. Scheduling generally consists of daytime hours, with flexibility to structure your route and visits to optimize efficiency and patient needs. Caseloads are designed to allow adequate time for thorough assessments, quality interventions, and meaningful education. Visit volumes and geographical assignment are managed to balance productivity with reasonable travel demands within the Denver metro, including a variety of neighborhoods and suburban communities.
