This role involves working within the field of home health, performing evaluations and treatments based on physician orders. Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluations and treatment of patients based upon physician orders/referrals.
  • Complete comprehensive evaluations using appropriate evaluation and assessment tools and procedures.
  • Review patient records for current diagnosis, past medical history, precautions, and contraindications.
  • Establish measurable goals and develop treatment plans to achieve maximum patient independence/function.
  • Regularly reassess the effectiveness of treatment plans and patient progress, modifying as needed.
  • Communicate patient needs and progress to the treatment team, physicians, and family members.
  • Consult with medical providers regarding patient evaluation, treatment progress, and discharge.
  • Recommend and assist in securing adaptive equipment to enhance rehabilitation programs.
  • Maintain and submit documentation required by company, facility, and regulatory standards.
  • Adhere to the company's Code of Business Conduct, professional practice standards, and applicable laws.
  • Supervise Physical Therapist Assistants and Aides in patient care activities in accordance with state practice act.
  • Participate in and/or conduct internal staff development programs and maintain clinical practice skills.

Requirements of the Physical Therapist
  • Valid KY compact state license
  • Experience in home health
  • Proficiency in HCHB, OASIS, and wound care
