Physical Therapist (PT) – Position Description

Location: York, Pennsylvania

Position Summary

The Physical Therapist is responsible for evaluating, planning, and implementing skilled therapeutic interventions to help patients improve mobility, restore function, reduce pain, and prevent disability. The PT works collaboratively with physicians, rehabilitation staff, and interdisciplinary healthcare team members to ensure high-quality patient outcomes in compliance with Pennsylvania state practice regulations.

Key ResponsibilitiesPatient Evaluation & Treatment
  • Conduct comprehensive physical therapy evaluations to determine functional deficits and patient needs
  • Develop individualized treatment plans using evidence‑based practices
  • Provide therapeutic interventions including exercise, manual therapy, functional training, and modalities
  • Measure progress and adjust care plans to ensure optimal outcomes
  • Educate patients and families on home exercise programs, mobility, and safety techniques
Clinical Documentation & Compliance
  • Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and in a timely manner
  • Maintain compliance with state, federal, and facility regulations
  • Participate in quality improvement activities and adhere to professional practice standards
Collaboration & Communication
  • Coordinate care with physicians, nurses, occupational therapists, speech therapists, and other team members
  • Communicate effectively with patients, families, and caregivers
  • Participate in care conferences and team meetings as needed
Qualifications
  • Active Pennsylvania Physical Therapist license (in good standing)
  • Doctor of Physical Therapy (DPT) or Bachelor’s/Master’s degree in Physical Therapy from an accredited program
  • Current CPR/BLS certification
  • Experience in the specific setting (outpatient, acute, SNF, home health) preferred
  • Strong clinical reasoning, communication, and patient‑interaction skills
  • Ability to work independently and as part of a multidisciplinary team
