Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: Physical Therapy Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 1 year of Skilled Home Health Experience License: Requires Oregon and Washington licenses Certifications: BCLS required Must-Have: - Oasis experience required - Own transportation required - COVID vaccination and one booster required Description: Home Health Physical Therapist position available. This role involves providing care in home settings. The facility requires a 1:1 patient ratio. Therapists may visit up to 6 patients per day. The standard productivity points are 30 per day. Weekend work is not required. Travelers do not need to float between units. No block schedule accommodations are available. Local candidates will be accepted within a 50-75 mile radius. RTO restrictions should be confirmed with hiring management. The facility may call off up to one shift per pay period. Charting will be done using EPIC Remote Client for Home Health and Rover. Business casual attire is preferred, with scrubs allowed but not required. The patient demographics include adults and geriatrics. Diagnoses commonly encountered include dementia, Parkinson’s, ALS, and post-operative orthopedic conditions. The primary expectation will be to complete OASIS documentation. Additional procedures and duties may be required based on patient needs. Candidate must demonstrate effective communication and time management skills. Safety and injury prevention practices are essential. Candidates will be responsible for monitoring vital signs and conducting assessments. Providers are encouraged to utilize their skills in adaptive devices, gait training, and various therapeutic techniques. Experience in home health is preferred but not mandatory for first-time travelers. References covering one year of experience are required during the compliance process.
