Basic Cardiac Life Support (BCLS), Home Care, On Call, Physical Therapy, Reimbursement

Location: East Portland, OR and Clark County, WA surrounding areas
Schedule: Day shift, 8-hour shifts (08:00 - 16:30), 5 days per week. Up to 6 patient visits per day with a standard productivity expectation of 30 points. 1:1 patient ratio during visits. No weekend or on-call requirements.

Requirements:- Active Physical Therapy license in both Oregon and Washington (required)
- Minimum 1 year of skilled home health experience required
- BCLS (Basic Cardiac Life Support) certification
- OASIS documentation experience required
- Proficiency with EPIC Remote Client for Home Health and Rover phone application
- Reliable personal transportation required
- Two professional references covering 1 year within the last 3 years
