Job Overview
We are seeking a skilled Physical Therapist (PT) for a contract assignment in a rehabilitation/skilled nursing setting. The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, strength, balance, and functional independence.
The ideal candidate will work collaboratively with interdisciplinary teams to deliver high-quality, patient-centered rehabilitation care.
Key Responsibilities
Evaluate and assess patient rehabilitation needs
Develop and implement individualized physical therapy treatment plans
Provide therapeutic interventions to improve mobility and function
Monitor and document patient progress and outcomes
Educate patients and caregivers on therapy programs
Collaborate with physicians and rehabilitation teams
Ensure compliance with clinical and facility standards
Support patient recovery and functional independence goals
Requirements
Degree in Physical Therapy from an accredited program
Active Physical Therapist (PT) license in the state of Vermont
Rehabilitation or skilled nursing experience preferred
