Physical Therapist - Home Health - (PT - HH)

Available Shifts: 8 D
Pay Rate: $2692.00 - $2774.00
PT- home health needed asap
Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.
Bachelor's Degree or equivalent (some certifications will require Bachelor?s): Required
Master's Degree: Preferred
Physical Therapy licensure in all applicable states of practice: Required
Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Required
Valid Driver's License (DL) and must be at least 21 yrs of age or older: Required
Required Qualifications Physical Therapist, Home Health Licenses: PT-CA Certifications: BLS(Copy Needed) SSN Required DOB Required References: 1 Reference in entire work history
