Job Title: Home Health Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum of 1 year of Home Health experience License: Required Certifications: BLS (AHA) Must-Have: Home Health experience Description: Consideration will be given to candidates with less than 2 years of experience, provided they have Home Health experience. The unit specializes in therapy services, including physical therapy, occupational therapy, and speech-language pathology. The average daily census is 78 patients, which may increase with additional physical therapy availability. Physical therapists will conduct start of care visits, particularly for orthopedic patients when nursing is not ordered. The expected patient ratio is 5 visits per day. The electronic medical record system used is HomeCare HomeBase (HCHB). Direction and support will be provided by an intake registered nurse and a manager located in the office. Nurse aides are available to assist. There is no weekend or holiday requirement for this position. Scrub color is not specified. There are no ventilation support needs. No on-call responsibilities are assigned. Shifts are during the daytime. No floor-specific orientation is required. Common diagnoses include orthopedic joint replacements and geriatric patients with multiple comorbidities, such as CHF and COPD. Staff will be provided with tablets for electronic medical record documentation. The service area includes most of the designated county, with some remote regions outside the office's range. Therapists can expect to travel approximately 40 miles per day, although this may decrease with the addition of more therapy staff and a reallocation of patient load. The facility reserves the right to cancel up to 4 shifts during a 13-week assignment. This position does not require a BSN. The trauma level is not specified. The care setting is Home Health.
