Key Responsibilities of the Physical Therapist- Conduct SOC (Start of Care), evaluations, reassessments, and discharges
- Coordinate visit times with patients and manage daily schedules
- Collaborate with PTAs for routine visits
- Complete documentation in partner agency EMRs, including Pariox, Kantime, Kinnser (WellSky), Axxess, and Homecare Homebase
Requirements of the Physical Therapist- Valid Physical Therapy license
- Experience in home health settings preferred
- Proficiency with EMR systems
- Ability to cover Saturday orthopedic visits
